Compensation:

Overview:
Responsible for coordinating and delivering comprehensive medication management to enhance medication utilization.

Key Responsibilities:
1. Assess, interpret, and evaluate medication regimens to identify, prevent, and resolve potential or actual medication-related issues.


2. Deliver patient care by initiating, adjusting, and monitoring medication therapy through approved protocols, which may include therapeutic interchange and collaborative practice agreements.

3. Provide drug information and education to healthcare professionals, students, and patients.

4. Collaborate with the healthcare team to develop, implement, and assess quality improvement initiatives related to medication management.

5. Demonstrate critical thinking skills, sound judgment, and the ability to work independently in a fast-paced environment.

Qualifications:
1. Bachelor's Degree in Pharmacy from an accredited institution.

2. Valid state licensure as a Pharmacist and relevant life support certification as required.
