Crisis Services Nurse

Job Summary:
The Crisis Services Floater Nurse is a critical member of our team, providing direct medical care to clients in crisis settings, residential settings, or in case management roles. This role requires the ability to work independently and as part of a multi-disciplinary team to meet the needs of clients served.
Key Responsibilities:
- Provide direct medical care to clients in crisis settings, residential settings, or in case management roles.
- Assist with crisis interventions and medical emergencies.
- Coordinate with all team members regarding client discharge planning and serve as a liaison with other agencies as indicated.
- Complete charting for each client during every shift prior to the end of shift.
- Count controlled substances and document at every shift change.
- Obtain and chart vital signs on every shift.
- Monitor for significant changes in client's condition/behavior every shift and make referrals to obtain new orders as needed.
- Administer scheduled, routine, and PRN medication in a timely manner.
- Obtain blood sugars as ordered.
- Triage potential referrals with Mobile Crisis and Walk in Center.
- Coordinate arrival times for potential clients with Mobile Crisis, Walk in Center, and Transporters.
- Complete C-SSRS on each client.
- Attend to guest and/or client needs during medical emergencies (ie: seizures, falls, etc).
- Coordinate with Emergency Departments and EMS.
- Obtain Prior Authorizations.
- Answer phone calls from recently transitioned guests regarding medication issues.
- Weekly medication counts of routine medications.
- Update Admission/Discharge log each time a client is admitted or transitioned from unit.
- Complete follow-up calls on weekends in accordance with program requirements.
Requirements:
- LPN or RN degree with a current/valid Tennessee license.
- Course work or experience in the areas of cultural diversity, human development, primary care, etiology, and treatment of mental illness, alcohol and drug use, physical and sexual abuse, suicide, and intellectual developmental delays.
- Ability to present professionally, work within a team format, and have the mental ability to exercise sound, clinical judgment under pressure.
- Ability to communicate effectively (both oral and written) and possess good time management and organizational skills.
- Basic computer skills required; experience with electronic medical record (EMR) preferred.
- Experience with interdisciplinary collaboration and care coordination preferred.
Work Environment:
This position is based within Crisis Services, which provides services 24/7. The role of the Crisis Services Floater Nurse is to ensure that coverage is provided 24/7 despite any weather conditions that may arise. This position requires working different shifts as needed.
Travel:
Travel is expected for the float nurse as their assignments may differ on a day-by-day basis. The float nurse will be expected to travel between programs at times and may be required to travel for trainings or meetings. There is no overnight travel. Must have or be able to obtain an F-endorsement on driver license within first 2 weeks of employment.
Equipment/Technology:
Basic computer skills are required for email, timekeeping, and documentation in the electronic medical record. Must be familiar with basic Microsoft Office Skills. Must be able to use a fax machine and phone system.
