Direct Home Care Worker

PACE Southeast Michigan is seeking a skilled and compassionate Direct Home Care Worker to provide supportive services to our participants in their homes, communities, and day health centers.
Key Responsibilities:
- Assist participants with daily living activities, including personal care, meal preparation, and medication reminders.
- Provide emotional support and companionship to participants and their families.
- Assist participants with transportation and appointments.
- Document participant progress and communicate with healthcare teams.
- Respond to emergency situations and follow established policies and procedures.
Requirements:
- High School Diploma or equivalent required.
- Minimum 2 years of relevant work experience or completion of a formal training program.
- Ability to establish and maintain interpersonal relationships and respect participant dignity.
- Flexibility to work varied schedules and adapt to changing situations.
Working Conditions:
- Driving required within PACE Southeast Michigan catchment area.
- Reliable transportation and current valid driver's license required.
- Exposure to communicable diseases and potential for exposure to chemicals.
