Overview
The Behavioral Healthcare Coordinator plays a vital role in assisting the public in a warm and welcoming manner with requests for Behavioral Health Services. This position requires excellent customer service skills, empathy, and support to ensure individuals receive the necessary care and resources.
Key Responsibilities
- Welcome individuals in need and provide consultation to and coordinate patient care with health center primary care staff.
- Identify, refer, and advocate for patients needing specialty behavioral health services and other required services.
- Determine eligibility for Medicaid specialty/OCHN services and determine the appropriate level of care using clinical level of care tools.
- Conduct telephone follow-ups on patients deemed 'high risk' or 'at risk.'
- Utilize population health information to refer patients to targeted health and wellness services.
- Make referrals for patients deemed 'high-risk' and/or 'at risk' to receive appropriate screenings, exams, and annual preventative medical and behavioral health services.
- Participate in workgroups and committees with OCHN and Honor Community Health teams as deemed appropriate.
- Complete all charting/documentation as needed and required by contracted funding sources, including assessments, progress notes, and billing.
- Understand OCHN provider agencies and inform available choice of agency and services to those eligible for OCHN services.
- Maintain knowledge of multiple community resources for referral to individuals in need and/or do not meet criteria for OCHN services.
- Create new medical records for individuals seeking an Access Screening, if applicable, and completes initial documentation in medical record.
- Provide links and coordination with Crisis Providers for individuals experiencing crisis.
- Collaborate and coordinate as needed with internal and external providers.
- Prepare healthcare data reports and share with Access leadership in a timely manner, alerts leaders of detected trends or changes, and shares any identified barriers to achieving Access goals with Access leadership.
- Complete quarter and annual reporting for grant and submit to Access leadership.
Requirements and Qualifications
- Master's degree in a mental health field.
- Current State of Michigan license as Social Worker (LMSW), (LLMSW), or Counselor (LPC), (LLPC).
- One year minimum of Behavioral Health experience.
- Experience with Federally Qualified Health Centers is preferred.
- Preference for experience in the OCHN network.
