Pharmacist - Emergency Department/Transitions of Care Specialist

Job Summary:

This pharmacist will provide dedicated patient care, obtain medication histories, and oversee safe and efficient transitions of care from hospital admission to discharge. They will be primarily assigned to cover the Emergency Department and work within the hospital to provide rapid response and code response.

Key Responsibilities:
  • Interpret medication orders and prescriptions to ensure they are complete, safe, rational, and compatible with institutional policy.
  • Prepare compounds, including injectable products, and dispense medications for inpatient and ambulatory care use.
  • Perform duties in all areas of Pharmacy, including inpatient, ambulatory care, and satellites.
  • Provide information to healthcare practitioners to promote the rational use of medications.
  • Provide drug monitoring service and maintain Pharmacy records by law and institutional policy.
  • Interpret policy and procedures for hospital staff and participate in teaching programs for hospital staff.
  • Participate in Pharmacy Department's Quality Assurance Programs.
Requirements:
  • B.S. in Pharmacy or Pharm.D.
  • NYS license and registration, or eligibility.
  • Completion of PGY1 Pharmacy residency or equivalent experience.
Preferred Qualifications:
  • Previous hospital/emergency room experience.
  • Experience with transitions of care activities.
Work Schedule:

Overnights, with 7 on 7 off scheduling. Rotating shifts as necessary for the department.
