Job Summary:

The LPN will be responsible for conducting and monitoring medication calls and health calls as scheduled. You will work with clients and guardians to schedule medical appointments and provide follow-up care as needed. Additionally, you will work with the agency to ensure timely medication refills and assist with data entry and monitoring.

Responsibilities:
  • Conduct and/or monitor all medication calls and health calls as scheduled.

  • Provide on-site health services to address emerging and/or current health needs of clients.
  • Provide training and guidance to key staff members in emergency health situations.
  • Establish and monitor health-related policies and procedures to ensure a safe and healthy environment.
  • Provide immediate first aid in health emergencies and escort clients to emergency facilities as needed.
  • Conduct health assessments and make recommendations on client participation in treatment.
Requirements:
  • NYS Licensed Practical Nurse (LPN) certification.
  • Experience working in a substance abuse treatment setting or special populations.
  • Ability to work one of the schedule options listed above.
  • Flexibility in scheduling a plus.
  • Ability to work effectively in collaboration with diverse groups of people.
