Outreach Development Corporation is seeking a qualified Licensed Practical Nurse (LPN) to work in our men's residential facility located in Brentwood, Long Island. The PT LPN will be responsible for delivering and implementing patient health services as a member of the treatment team.
Responsibilities:- Conduct and/or monitor all medication calls and health calls as scheduled.
- Work with clients and guardians to schedule medical appointments and conduct follow-up as required.
- Work with the Agency regarding client medication and pharmacy refills to ensure prescriptions are filled/re-filled in a timely manner.
- Provide on-site health services to address emerging and/or current health needs of clients.
- Provide training and guidance to key staff members in emergency health situations.
- Establish and monitor health-related policies and procedures to ensure a safe and healthy environment.
- Escort clients to emergency and/or health facilities when necessary.
- NYS Licensed Practical Nurse (LPN) certification.
- Experience working in a substance abuse treatment setting or special populations.
- Ability to work one of the schedule options listed above.
- Flexibility in scheduling a plus.
- Ability to work effectively in collaboration with diverse groups of people.
