Job Summary:

We are seeking a skilled Community Care Worker I to join our team at the Texas Department of Aging & Disability Services. As a key member of our team, you will play a vital role in determining the needs and eligibility of individuals for Community Care services.

Key Responsibilities:
  • Conduct home visits and interviews with individuals to assess their needs and eligibility for services.
  • Document information, determine need for services, and develop service plans.
  • Verify data, explain program benefits and requirements, and refer individuals for appropriate services.
  • Authorize and monitor CCAD services to ensure they meet the individual's needs.
Essential Job Functions:
  • Attend work on a regular and predictable schedule and perform other duties as assigned.
  • Interview applicants, individuals receiving services, and resource persons, and conduct home/site visits to assess the need for services.
  • Conduct reviews to determine eligibility and functional needs.
  • Maintain case record documents and enter information in an automated system.
  • Monitor services to determine if they are meeting the individual's needs.
  • Develop and coordinate service plans with individuals, families, provider agencies, and other state agency staff.
Knowledge, Skills, and Abilities:
  • Ability to effectively communicate orally and in writing.
  • Ability to establish and maintain effective relationships with individuals, co-workers, and staff from other agencies.
  • Ability to operate computer and general office equipment.
  • Knowledge of interviewing techniques and resources that serve individuals who are older or have a disability.
