Job Title: Substance Use Counselor

VNS Health is seeking a skilled Substance Use Counselor to join our team in Brooklyn, New York.

About the Role:

We are looking for a compassionate and experienced Substance Use Counselor to provide high-quality care to our clients. As a Substance Use Counselor, you will work collaboratively with our interdisciplinary team to develop and implement individualized treatment plans that address the unique needs of each client.

Key Responsibilities:
  • Conduct phone and field outreach to locate and enroll clients into programs and services
  • Build relationships and trust with clients and their families, and assist with their interactions with professionals on the team
  • Work collaboratively on an interdisciplinary team to discuss care needs and identify solutions to support clients
  • Apply mutually shared and lived experiences to build relationships and trust with clients
  • Educate clients about program services, benefits, and self-help techniques
  • Advocate for effective recovery-based services on behalf of clients
  • Teach and model symptom management and coping skills for resilience
  • Review service plans with clients and their families, and provide ongoing education, guidance, support, and encouragement
  • Develop inventory of resources that meet the client's needs
  • Provide navigation services to help clients connect with community-based services and supports
  • Document in EMR in accordance with program policies, procedures, VNS Health standards, and city, state, and federal regulatory requirements
Requirements:
  • High school diploma or equivalent required
  • Minimum of one year experience in a mental health, substance use treatment program, healthcare, or human services setting, preferred
  • Experience working with a severely mentally ill, psychological trauma, and/or substance using population, preferred
  • Effective oral, written, and interpersonal communication and relationship building skills required
  • Ability to work independently and collaboratively on an interdisciplinary team
  • Computer literacy (electronic health records, word processing, email, internet research, data entry), required
  • Valid New York State driver's license, as determined by operational/regional needs
  • Bilingual skills in English and Spanish, preferred
