As a Home Health Physical Therapist, you will work with patients in their homes to provide physical therapy services and help them achieve their goals.

Schedule: Monday - Friday, 8:15AM - 5:00PM

Territory: Riverside, Beavercreek, Kettering, Centerville, Waynesville, Bellbrook, Spring Valley, Centerville

Sign on Bonus: $20,000

Key Responsibilities:

  • Provide skilled physical therapy services to patients in their homes
  • Develop and implement individualized plans of care to meet the unique needs of each patient
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Communicate effectively with patients, families, and healthcare teams
  • Maintain accurate and timely documentation of patient progress and care

Requirements:

  • Current license as a Physical Therapist in the state of employment
  • Valid driver's license and auto insurance
  • Ability to work independently and as part of a team
