Job Title: Home Health Physical Therapist

We are seeking a skilled and compassionate Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes.

Job Summary:

The successful candidate will be responsible for providing skilled physical therapy services to patients in their homes, as ordered by the attending physician. This includes evaluating patients, developing and implementing individualized plans of care, and educating patients and their families on proper use of equipment and techniques.

Responsibilities:
  • Provide skilled physical therapy services to patients in their homes, as ordered by the attending physician.
  • Evaluate patients to determine their physical therapy needs and develop individualized plans of care.
  • Implement and modify plans of care as needed to ensure optimal patient outcomes.
  • Educate patients and their families on proper use of equipment and techniques to promote independence and safety.
  • Document all aspects of patient care, including evaluations, treatments, and progress notes.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Participate in ongoing education and training to stay current with best practices and industry developments.
Requirements:
  • Current licensure as a Physical Therapist in the state of employment.
  • Minimum of one year of experience as a Physical Therapist in an acute setting.
  • Home care experience is desired, but not required.
  • Ability to effectively communicate and create positive impressions with patients, families, and healthcare team members.
  • Knowledgeable on therapy best practices and industry developments.
  • Ability to make appropriate physical therapy judgments and adapt to changing patient needs.
