We are seeking a skilled and compassionate Home Health Physical Therapist to join our team at Alternate Solutions Health Network LLC. As a Home Health Physical Therapist, you will play a vital role in providing high-quality care to patients in their homes, helping them to recover from illness or injury and regain their independence.

Job Summary:

The Home Health Physical Therapist will be responsible for assessing patients' physical abilities, developing and implementing individualized plans of care, and providing skilled physical therapy services to patients in their homes. The ideal candidate will have excellent communication and interpersonal skills, be able to work independently, and have a strong commitment to providing exceptional patient care.

Responsibilities:
  • Assess patients' physical abilities and develop individualized plans of care
  • Provide skilled physical therapy services to patients in their homes
  • Communicate effectively with patients, families, and healthcare teams
  • Document patient progress and outcomes
  • Collaborate with other healthcare professionals to ensure comprehensive care
Requirements:
  • Current licensure as a Physical Therapist in the state of employment
  • Minimum of one year of experience as a Physical Therapist in an acute setting
  • Home care experience preferred
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
