About the Role:

Key Responsibilities:

  • Work closely with healthcare providers to formulate customized treatment strategies that prioritize the client's home safety and comfort.
  • Evaluate patients' physical capabilities, including strength and mobility, to determine suitable therapeutic interventions.
  • Keep the nursing team and physicians informed about patient progress and any significant changes in condition.
  • Develop and execute individualized physical therapy programs, offering support and education to clients' families and caregivers.
  • Maintain accurate clinical documentation and engage in discharge planning processes.
  • Foster respectful and empathetic relationships with clients, their families, and your colleagues.

Qualifications:

  • A Master's degree or higher in Physical Therapy from an accredited institution.
  • Successful completion of the National Physical Therapy Examination (NPTE) along with state-specific licensing requirements.
  • Active state licensure to practice as a Physical Therapist.
  • A minimum of two years of experience in physical therapy, ideally within community or home health environments.
  • Adherence to HIPAA regulations and a strong commitment to maintaining patient confidentiality.
