Position Overview:

PeaceHealth is in search of a Registered Nurse specializing in hospice care for a Full Time, 1.00 FTE, Variable role. The compensation range for this position is $40.09 – $76.12.
The starting salary is influenced by various factors, including education, training, work experience, and any applicable collective bargaining agreements.

Key Responsibilities:
This role entails delivering direct skilled nursing services and overseeing the management of patients enrolled in hospice or clinic programs, employing significant judgment and expertise in patient care decisions.

Utilizes a professional healthcare approach involving assessment, problem identification, planning, intervention, and evaluation of clinical outcomes.

Specific Duties Include:
1. Conducting initial evaluations of patients and their homes to determine eligibility for home health or hospice services.
2. Providing skilled nursing care and education to patients and their caregivers in accordance with the established treatment plan.
3. Performing comprehensive physical assessments and developing, coordinating, and managing individualized care plans as directed by the primary physician to achieve both short and long-term medical objectives.

4. Developing and managing the Home Health Aide's care plan based on the patient's evolving needs.
5. Coordinating care and collaborating with interdisciplinary team members to ensure a seamless continuum of care.

Qualifications:
- Graduate of an accredited nursing program is required; a Bachelor of Science in Nursing (BSN) is preferred.
- A minimum of one year of experience in home health or hospice nursing is required; hospice experience is preferred.
- Current Registered Nurse licensure in the state of practice is mandatory.
- Current Basic Life Support (BLS) certification from the American Heart Association is required.
- A Washington Food Handler's Card is necessary at the time of hire.

This position is subject to a collective bargaining agreement, and there may be multiple openings available.
