Clinical Pharmacist

We are seeking a highly skilled Clinical Pharmacist to join our team at GPAC. As a Clinical Pharmacist, you will play a critical role in providing medication therapy management services to patients in collaboration with other healthcare providers.
Responsibilities:- Assess and review patients' medical status to advise on prescribed medication and optimize patient outcomes.
- Provide oversight of pharmacy technicians and serve as a reference for medical, nursing staff, and patients.
- Work collaboratively with an interdisciplinary team to maximize patient outcomes.
- Perform and document medication reconciliation, comparing prescriber's orders to the patient's active medication list(s).
- Reconcile discharge medication orders with inpatient pharmacy profile and admission medication history and perform discharge counseling.
- Ensure that patient medication regimes are consistent with all WCHS medication use guidelines and policies and formulary.
- Medication Therapy monitoring and coordination of pharmacy receiving, dispensing, and distribution.
- Prepare and/or check all non-sterile and sterile compounds.
- Doctorate in Pharmacy.
- Current Wyoming Pharmacy License.
- Exceptional knowledge of medical terminology and processes.
- Basic Life Support (BLS) Certification.
- Clinical pharmacy and orders management experience preferred.
