Job Overview

We are seeking a skilled Home Health Therapist Specialist to join our team at DCH Health System. This role involves providing physical therapy services to patients in their homes, ensuring they receive the best possible care and support.

About the Job
  • Evaluate patient needs and develop individualized care plans.
  • Provide therapeutic procedures and treatments to patients in their homes.
  • Collaborate with other healthcare professionals to ensure comprehensive care.
  • Maintain accurate records and documentation of patient care.
  • Participate in ongoing education and training to stay current on best practices.
Requirements
  • Bachelor's or Master's degree in Physical Therapy.
  • Licensed as a Physical Therapist and maintaining active status with the state board or licensing agency.
  • Current BLS certification or ability to obtain within 60 days of employment.
  • Working knowledge of computer systems and excellent communication skills.
  • Detail-oriented and able to work with minimal supervision.
Work Environment
  • Prolonged standing and walking during home visits.
  • Assisting patients with turning and positioning up to 200 lbs.
  • Repeated bending, squatting, and stooping during treatment sessions.
Compensation and Benefits
