Family Housing First Case Manager Position

Job Summary: The Family Housing First Case Manager provides home and community-based case management support to families who have experienced chronic homelessness. This role requires a deep understanding of the Housing First model and a harm reduction approach to ensure clients have the support and services necessary to maintain positive tenancies.

Key Responsibilities:

  • Conduct client interviews and assessments, make collateral contacts, and consult with other providers to ensure comprehensive service delivery.
  • Evaluate clients' capacity to make independent living decisions and assess risks to clients and other tenants at the housing site(s).
  • Develop and modify individual client service plans quarterly to assess ongoing needs for clinical, supportive services, and housing needs.
  • Maintain regular contact with clients through home visits, phone calls, and office visits to ensure their safety and well-being.
  • Identify, coordinate, and advocate for appropriate support services and resources for each client, providing linkages to services and resources as needed.
  • Work cooperatively with landlords where clients are residing to ensure a smooth and supportive living environment.
  • Maintain accurate case records, enter and update information on clients served, in internal and external databases, in a timely fashion, meeting all mandatory deadlines.

Requirements:

  • A BS degree in Human Services or a related field or three years of relevant work experience.
  • A valid driver's license and reliable transportation.
  • Ability to work with clients in crisis and those actively engaging in substance use.
  • Ability to practice effective de-escalation techniques and take reasonable precautions to ensure personal safety.
  • Reliable, responsible, highly organized, and willing to take initiative.
  • Excellent organizational, interpersonal, and communication skills.
