Key Responsibilities:- Provide personalized patient care in the home setting, working closely with physicians and other healthcare professionals to develop and implement effective care plans.
- Conduct thorough assessments, document progress, and report changes to physicians to ensure seamless care coordination.
- Educate patients and their families on medication management, therapy, diet, and exercise to promote optimal health outcomes.
- Collaborate with a team of healthcare professionals to ensure comprehensive care and support.
- Active Registered Nurse (RN) License in Colorado
- At least 1200 hours of experience as an RN
- Healthcare CPR/BLS Certification
- Home health care experience preferred
