Nurse Care Coordinator

Tidelands Health is seeking a skilled Nurse Care Coordinator to join our team. As a Nurse Care Coordinator, you will play a critical role in ensuring that patients receive the best possible care. Your primary responsibility will be to coordinate care activities, such as pre-certifications and lab results follow-up.
Key Responsibilities
This role involves:
• Coordinating care activities, including pre-certifications and lab results follow-up
• Ordering and maintaining pharmaceutical sample supply program
• Performing nurse visits and patient education
• Triaging incoming phone calls
Requirements
To be considered for this position, you will need:
• A minimum of 2 years' experience as an LPN
• Strong communication and interpersonal skills
• Ability to work effectively in a fast-paced environment
• Light Physical Agility Test (PAT) rating
Salary Estimate
$50,000 - $70,000 per year
