Job Summary

We are seeking a compassionate and skilled LVN to join our team in Cedar Hill, TX. As a Pediatric Home Health Nurse, you will provide high-quality care to our patients, working closely with our multidisciplinary team to develop and implement individualized care plans. Responsibilities
  • Provide one-on-one care to pediatric patients with medical complexities
  • Develop and implement individualized care plans in collaboration with our multidisciplinary team
  • Work closely with patients and families to ensure seamless care transitions
  • Participate in ongoing education and training to stay up-to-date on best practices in pediatric home health care
Requirements
  • LVN licensure in the state of Texas
  • Experience working with pediatric patients with medical complexities
  • Strong communication and interpersonal skills
  • Ability to work independently and as part of a team
