Key Responsibilities:- Collaborate with patients, families, and healthcare providers to develop and implement personalized care plans
- Conduct comprehensive assessments and provide ongoing care to patients with complex medical needs
- Work closely with our interdisciplinary team to ensure seamless care coordination and communication
- Participate in quality improvement initiatives to enhance patient outcomes and experience
- Stay up-to-date with the latest medical research, technology, and best practices in home-based care
- Completion of a Nurse Practitioner or Physician Assistant accredited program
- Current state RN license (if Nurse Practitioner) and NP or PA license
- Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
- Knowledge of long-term care, community resources, and cost-effective alternatives regarding patient care delivery systems
