Job Title: Hospice Aide-PRN

At Freedom At Home Dialysis, we are committed to providing exceptional care to our patients. We are seeking a compassionate and skilled Hospice Aide-PRN to join our team.

Job Summary:

The Hospice Aide-PRN will provide direct patient care under the direction of the RN and according to the Aide Plan of Care. This includes assisting patients with safe transfers and ambulation, reporting changes and documenting pertinent information, and promoting a clean, safe, and comfortable environment.

Responsibilities:
  • Provide direct patient care under the direction of the RN and according to the Aide Plan of Care.
  • Assist patients with safe transfers and ambulation per the Hospice policy.
  • Report changes and document pertinent information and care rendered to patient to ensure continuity of care.
  • Practice accepted infection control principles and provide a clean, safe, and comfortable environment.
  • Promote positive, supportive, respectful communication to patient/family and other employees.
  • Contribute to the management and efficient operation of the Hospice and demonstrate effective time management skills.
  • Perform treatments and procedures according to the Hospice policy.
  • Promote the Hospice philosophy and administrative policies to ensure quality of care.
Qualifications:
  • High School Diploma Preferred.
  • Has successfully completed one of the following: A) Training program/competency evaluation as specified in §418.76(b)(c) of the Medicare regulations; B) Competency evaluation program that meets §418.76( c); C) Nurse aide training/competency evaluation program approved by the state and listed in good standing on the state nurse aide registry; D) State Licensure program that meets requirements of §418.76(b).
  • Must have a current CNA and drivers license.
  • At least one year experience preferred as a Hospice Aide or Nursing Assistant in a hospital, nursing home, home health agency, hospice or long-term care facility and completion of Certified Nursing Assistant or Certified Home Health Aide Program.
Compensation:

$25 per visit
