Key Responsibilities

The Hospice Aide will work under the guidance and continuous supervision of a registered nurse, delivering personal direct patient care services as outlined in the Plan of Care on a daily basis.
  • Communicates with the Executive Director or designated personnel after each patient visit regarding the patient's condition or living situation, adhering to the Care Management process as necessary.
  • Assists with essential Activities of Daily Living (ADLs) during each patient interaction, including mobility, transfers, grooming, bathing, dressing, eating, and toileting.
  • Performs necessary household tasks that are vital to the patient's health care at home, ensuring services are provided to prevent or delay the need for institutional care.
  • Engages in the QAPI (Quality Assessment & Performance Improvement) initiatives, identifying issues and reporting to the administrator as required.
  • Must possess reliable transportation.

Qualifications

Licensure Requirements

  • Completion of a competency evaluation prior to providing patient care.
  • Current CPR certification.
  • Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.

Additional State-Specific Requirements

  • States such as AZ, FL, KS, LA, MI, MN, PA, SC, VA, WV: No further requirements.

Skills Required

  • Ability to handle the demands of a hospice aide role with maturity and professionalism.
  • Proficient in reading, writing, and following instructions accurately and promptly.
