Key Responsibilities
The Hospice Aide, under the guidance and continuous oversight of a registered nurse, delivers personal direct patient care services as outlined in the Plan of Care on a daily basis.
- Communicates with the Executive Director or designated personnel after each patient visit regarding the patient's condition or environment, adhering to the Care Management process as necessary.
- Assists with Activities of Daily Living (ADLs) during each patient interaction, including mobility, transfers, grooming, bathing, dressing, eating, and toileting.
- Performs essential household tasks that support the patient's health care at home, ensuring services are conducted to prevent or delay the need for institutional care.
- Engages in the Quality Assessment and Performance Improvement (QAPI) process, identifying issues and reporting to the administrator as needed.
- Must possess reliable transportation.
Licensure Requirements
- Completion of a competency evaluation prior to patient care.
- Current CPR certification.
- Valid driver's license, vehicle insurance, and access to a reliable vehicle or public transportation.
- No additional requirements for states including AZ, FL, KS, LA, MI, MN, PA, SC, VA, WV.
- Ability to effectively manage the demands of a hospice aide role.
- Proficient in reading, writing, and following instructions accurately and promptly.
