As a Case Management Coordinator at House of Mercy Inc., you will play a vital role in helping individuals living with HIV/AIDS overcome challenges and achieve stability in their lives. With a focus on providing person-centered trauma-informed services, you will assist clients in reaching optimal health and maintaining stable housing.

Your responsibilities will include conducting initial psychosocial assessments, developing individualized care plans, linking clients with community resources, and facilitating group meetings. You will also ensure that clients have access to safe permanent housing and provide crisis intervention as needed.

This is a full-time position offering a competitive salary of $$55,000 - $65,000 per year, plus benefits including medical, dental, and life insurance, flexible spending accounts, and a 403(b) retirement plan.
