Job Summary

We are seeking a skilled and compassionate Home Health RN to join our team at HCA Midwest Healthcare at Home. As a key member of our interdisciplinary team, you will provide high-quality, patient-centered care to individuals in the comfort of their own homes.

Responsibilities
  • Assess patients' physical, psychosocial, and environmental needs, and develop individualized plans of care to address these needs.
  • Provide skilled nursing care, including wound care, medication management, and education to patients and their families.
  • Collaborate with the interdisciplinary team, including physicians, therapists, and social workers, to ensure comprehensive care and achieve patient goals.
  • Supervise and provide clinical direction to home health aides and LPNs/LVNs to ensure quality and continuity of services.
  • Participate in on-call rotation and emergency call according to agency policy.
Requirements
  • Basic Cardiac Life Support certification within 30 days of employment start date.
  • Registered Nurse licensure in the state of employment.
  • Associate Degree or Registered Nurse Diploma.
  • At least one year of experience as a Home Health RN.
