Nurse Practitioner

Tri-FLEXSI Home Health Care, Inc. is seeking a highly motivated and compassionate Registered Nurse (RN) to provide high-quality home care services to patients in the Greater Houston Area. As a skilled nursing professional, you will be responsible for planning, organizing, and directing home care services, with a focus on community health education and experience.
Key Responsibilities- Conduct initial assessments of patients and families to determine home care needs
- Provide complete physical assessments and histories of current and previous illnesses
- Regularly re-evaluate patient nursing needs and make necessary revisions to the plan of care
- Develop and implement care plans that establish goals based on nursing diagnoses and incorporate therapeutic, preventive, and rehabilitative nursing actions
- Administer medications and treatments as prescribed by physicians
- Counsel patients and families in meeting nursing and related needs
- Provide health care instructions to patients as appropriate per assessment and plan of care
- Identify discharge planning needs and implement prior to discharge of patients
- Act as Case Manager when assigned by Clinical Supervisor and coordinate patient care for assigned caseloads
- Registered nurse with current licensure to practice professional nursing in the state
- Graduate of National League for Nursing accredited school of nursing
- Maintains a current CPR certification
- Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order
- Minimum of 2 years experience, at least one of which is in the area of public health or home care nursing
- Competitive hourly pay of $55 to $100 per visit, paid semi-monthly
