Job Title: Registered Nurse - Home Health

VNS Health is seeking a skilled Registered Nurse to join our team as a Home Health professional. As a member of our interdisciplinary care team, you will be responsible for providing high-quality patient care and support to individuals in their own homes.

Job Summary:

The Registered Nurse - Home Health will work closely with our team to assess the needs of patients, develop personalized care plans, and provide education and support to promote healthy living and independence.

Responsibilities:
  • Conduct comprehensive assessments of patients' physical and emotional needs
  • Develop and implement individualized care plans to meet patients' needs
  • Provide education and support to patients and their families on health management and self-care
  • Collaborate with other healthcare professionals to ensure seamless care coordination
  • Document patient progress and outcomes
Requirements:
  • Current registration as a Registered Professional Nurse in New York State
  • Associate's Degree in nursing required; Bachelor's Degree in nursing preferred
  • Prior experience working with an aging population and providing clinical assessments of their health needs
