Job Title: Registered Nurse - Home Health

VNS Health is seeking a skilled Registered Nurse to join our team in providing high-quality home health care services to our patients.

About the Role:

As a Registered Nurse in our Home Health program, you will be responsible for delivering comprehensive care to patients in their homes, focusing on their physical, emotional, and social well-being.

Key Responsibilities:
  • Conduct thorough assessments of patients' health needs and develop personalized care plans.
  • Provide direct patient care, including administering medications, wound care, and other treatments as needed.
  • Collaborate with interdisciplinary teams to ensure seamless care coordination and communication.
  • Monitor patients' progress, adjust care plans as necessary, and document all interactions and interventions.
  • Participate in quality improvement initiatives and contribute to the development of best practices in home health care.
Requirements:
  • Current licensure as a Registered Nurse in New York State.
  • Associate's or Bachelor's degree in nursing.
  • Experience working with older adults and individuals with complex health needs.
  • Strong communication and interpersonal skills.
  • Ability to work independently and as part of a team.
