**Nurse Preceptor

VNS Health is seeking a skilled and experienced Registered Nurse to join our team as a Home Health Preceptor. As a Preceptor, you will play a vital role in supporting the onboarding of newly hired RNs and LPNs, providing individualized instruction and guidance during the transition period.

• Act as a Preceptor for newly hired field RNs and LPNs, providing individualized instruction and guidance during the onboarding period.
• Provide ongoing feedback to branch management regarding individual nurse's learning progress and performance.
• Use a variety of tools to identify, document, and address orientation, performance, learning progress, and ongoing needs.
• Conduct and facilitate preceptor team meetings to promote effective communication and decision-making.
• Support the knowledge and skills development of new RNs and LPNs, individually and in small groups.
• Make joint visits with field clinicians to observe and instruct in current/new techniques and procedures.
• Provide nursing care in accordance with VNS Health policies, practices, procedures, and Standards of Nursing Practice.
• Act as a subject matter expert to staff for information concerning patient care.
• Participate in selected student programs to enhance professional knowledge and provide training/guidance to staff.
• Participate in selected activities to various community audiences in conjunction with clinical operations.
• Provide input and feedback for continuous improvement of the onboarding process for new RNs and LPNs and learning/education needs for incumbent staff.
• Assume responsibility for personal professional growth through attendance at in-services, internal and external training/workshops/conferences, select journal readership, and continued formal education.

• A license and current registration to practice as a Registered Professional Nurse in New York State.
• Valid driver's license.
• OASIS certification preferred.
• Certification in Hospice and Palliative Care Nursing preferred.
• Four years' clinical home care experience required.
• Minimum of two years full-time clinical home care experience required.
• Minimum of one year of experience as a field staff nurse in VNS Health Home Care.
