About the Role:
We are seeking a dedicated and compassionate Registered Nurse to join our team at Helping You Heal LLC. As a Nurse Practitioner - Community Care Specialist, you will play a vital role in providing high-quality patient care and support services to individuals with Serious Mental Illness (SMI).
Key Responsibilities:
- Conduct assessments and develop individualized treatment plans in collaboration with the interdisciplinary team.
- Provide medication management and administration, including oral and intramuscular psychotropic medications.
- Monitor patients for medical problems and side effects, and engage in health promotion and education activities.
- Communicate and coordinate services with other medical providers, and educate the team on monitoring psychiatric symptoms and medication side effects.
- Collaborate with the team psychiatrist to coordinate medical assessments, referrals, and treatment plans.
Qualifications:
- Registered Nurse (RN), Licensed Practical Nurse (LPN), or Nurse Practitioner licensure.
- Minimum of one (1) year experience working with adults with serious mental illness and a working knowledge of psychiatric medications.
