About the Role

We are seeking a skilled Mental Health Case Manager to join our team of dedicated professionals. As a key member of our rehabilitation services team, you will work independently in the field to provide support and guidance to individuals with mental illness.

The ideal candidate will have a Bachelor's degree and 2 years of experience in case management. They will be responsible for engaging with clients, assisting them in meeting community living demands, and linking them to relevant services to promote recovery and independence.

Key Responsibilities:

  • Conduct assessments and develop personalized service plans for clients
  • Facilitate group meetings on wellness management and provide regular coaching sessions
  • Collaborate with clients to set and achieve rehabilitation goals
  • Maintain accurate records and reports on client progress

Requirements:

  • Bachelor's degree in a related field
  • Minimum 2 years of experience in case management
  • Excellent communication and interpersonal skills
