Job Summary:
This is an exciting opportunity for a licensed mental health professional to join Scismic Inc's innovative Safe Options Support (SOS) team, which provides comprehensive care to street homeless or subway dwelling individuals. The role involves community outreach, coordinating participants' needs, enhancing daily living skills, providing supportive counseling, and advocating on their behalf.

Responsibilities:
* Persistent and assertive outreach and engagement using strength-based approaches
* Partnering with current street outreach teams, local police precincts, hospitals, MTA, Department of Homeless Services, and family members/caregivers to identify those in most need of outreach and care
* Continuously assessing the health and social needs of participants through SOS's conversational and observational assessments and formalized risk assessments tools
* Collaborating with the centralized SOR Hub to identify available housing and supporting participants through the process
* Participating in hospital discharge planning meetings to identify the best community resources for returning patients
* Providing short-term therapeutic counseling and support to participants pre- and post-housing
* Supervising case managers and peers on the SOS team
* Collecting and reporting data, as required, and working with the team leader, data analyst, and other SOS teams to use data to inform future care delivery

Requirements:
* Master's degree or higher in social work, mental health counseling, nursing, or psychology
* Licensed social worker, mental health counselor, therapist in New York State
* Experience working with homeless and/or precariously housed populations preferred but not required
* Knowledge of homeless resources, NYC shelter systems, and MTA transit systems a plus

Compensation:
The salary for this position is $74,984, plus generous benefits.
