Job Title: Navigation Program Manager

Cocoon House is seeking a highly skilled and compassionate Navigation Program Manager to support our Navigation program, which aims to prevent and resolve homelessness among young adults aged 18-24. As a key member of our team, you will work closely with young adults, program staff, and community partners to ensure the effective delivery of services and support.

Key Responsibilities:
  • Lead the Coordinated Entry team and ensure the accurate and timely response to referrals from multiple sources.
  • Collaborate with program staff to navigate challenging cases and situations, including emergency consultations and crisis intervention.
  • Ensure all grant requirements and outputs are met, including accurate and thorough documentation, data entry, and file audits.
  • Develop and maintain relationships with community partners, including law enforcement, resource centers, schools, and other organizations, to identify and target young adults and families in need of services.
  • Support the development of the Snohomish County Youth and Young Adult Coordinated Entry system, including creating and maintaining a sustainable regional CE structure.
  • Attend meetings with the County and partner agencies to maintain communication, stay current on funding requirements, and coordinate efficient service delivery.
  • Meet with young adults experiencing homelessness to enroll them in Coordinated Entry, maintain a small caseload, and provide case management to connect households to resources and housing.
  • Inspire and motivate team members, using a strengths-based approach to support staff through challenging situations.
Requirements:
  • High School Diploma or GED required, with a minimum of one year of experience working directly with young adults experiencing homelessness or any combination of experience and education that supports the needs of the position.
  • Spanish-speaking/multi-lingual skills preferred.
  • Strong communication and crisis intervention skills, with the ability to effectively assess situations and access emergency services when necessary.
  • Valid Washington State driver's license, insurance, and reliable transportation required.
  • Clear background check with DCYF required.
  • Excellent organizational skills, with basic data recording skills and record-keeping abilities.
  • Knowledge of street culture, adolescent development, trauma-informed care, and strengths-based approaches.
