Key Responsibilities
  • Conduct outreach activities, including phone calls, home visits, and community presentations, to educate clients about program options and eligibility requirements.
  • Build and maintain relationships with community, state, and faith-based organizations to enhance our presence in the community.
  • Provide one-on-one and group outreach presentations to clients and community members within a specific territory or initiative.
  • Assist clients in enrolling, disenrolling, and transferring into health plans, as needed.
  • Retrieve, research, and analyze data from multiple databases to answer client concerns and complete direct data entry into designated internal automated systems.
  • Identify issues, problem-solve, and escalate unresolved issues and concerns to management immediately and regularly.
  • Establish and maintain knowledge of community events, health fairs, and other activities, as needed.
  • Multi-task in several computer applications at once while holding conversations with clients.
  • Document activity in automated systems and report outcomes on a daily/weekly basis.
  • Ensure confidentiality of client information.
Requirements
  • High School diploma or equivalent with 2 years of human services experience or experience dealing with the public.
  • DSHS Community Health Worker certification preferred.
  • Proficient in Microsoft Office products, including Excel.
  • Excellent organizational, written, and verbal communication skills.
  • Ability to work in a fast-paced, deadline-oriented work environment.
  • Ability to work as a team member, as well as independently.
  • Reliable transportation with proper insurance and a Texas Driver's License that meets the State of Texas requirements.
