About the Position

We are seeking a skilled Nurse Practitioner or Physician Assistant to join our team at Matrix Medical Network. As a Home Risk Assessment Clinician, you will conduct comprehensive health assessments in the comfort of patients' homes, providing unmatched insight into their overall health and well-being.

Our Comprehensive Health Assessment helps to improve quality of care and allows us to potentially close multiple care gaps with a single visit. You will work collaboratively with physicians, case managers, social workers, family members, and key caregivers to provide patient-centered care.

Responsibilities

  • Conduct comprehensive health assessments in patients' homes, reviewing and observing their current health, medical history, medication adherence, social environment, and other risks.
  • Work collaboratively with healthcare teams to provide patient-centered care, including physician referrals, case management referrals, and follow-up.
  • Develop and implement personalized care plans to address patients' unique needs and health goals.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.

Qualifications

  • Master's Degree in Nursing or Physician Assistant Studies, or commensurate experience and licensure.
  • Current RN and NP/PA licensure in the state of practice, including prescription authority.
  • Board certification in a Matrix-approved specialty.
  • Minimum 1 year experience as a Nurse Practitioner or Physician Assistant.
  • Strong computer skills and familiarity with EMR software.
  • Excellent verbal and written communication skills.
