Job Title: Transitioned Aged Youth Specialist

Polara Health is seeking a skilled Transitioned Aged Youth Specialist to join our team. As a key member of our staff, you will play a vital role in empowering young adults with disabilities to achieve a successful transition to adulthood.

Job Summary

The Transitioned Aged Youth Specialist will work closely with our team to provide pre-employment training, education, and one-on-one skill building to young adults with disabilities. Your primary goal will be to assist these individuals in reaching and maintaining their highest level of independence.

Key Responsibilities
  • Assist youth with disabilities in reaching and maintaining their highest level of independence through individualized skills training
  • Teach empowerment, self-advocacy, and disability pride to youth in individual and group settings
  • Mentor youth and provide opportunities for growth and leadership
  • Provide information and support to assist clients in achieving stated goals and/or refer the client to appropriate resources
  • Establish and maintain relationships with community partners
  • Work with individuals to develop treatment plans goals specific to Transitioned Aged Youth (TAY) services
  • Maintain all required documentation
  • Report on successes, progress toward grant goals, and program activities
  • Assist with developing engaging, age-appropriate curriculum for students to prepare them for transition into adulthood
  • Attend quarterly meetings with state programs and complete deliverables in a timely manner
  • Carry out additional assignments as delegated by supervisors in a timely manner
Qualifications

To be successful in this role, you will need to possess the following qualifications:

  • 21 years of age or older; or minimum 18 years old and licensed or certified under A.R.S. Title 32 and providing services within the personnel member's scope of practice; or minimum 18 years old for select administrative positions
  • Must be able to provide or obtain an AZDPS Level One Fingerprint Clearance Card
  • Must possess a valid Driver License
  • Minimum of a High School Diploma or GED required. Bachelor's degree preferred.
  • Minimum 1-2 years' experience in similar field or combination of education/experience equal to same.
Working Conditions

This role will require working in an office environment, with occasional travel within the community as needed. You will be required to use computers, cell phones, and other office equipment extensively. You must be able to sit for prolonged periods and occasionally lift and/or move 5 or more pounds. You will also be required to respond appropriately and effectively in crisis situations by using good judgment and following Polara Health protocols.
