The Rehabilitation Specialist is tasked with conducting comprehensive assessments, designing and implementing therapeutic interventions, and maintaining detailed documentation in alignment with the individualized care plan established for each client.


Key Responsibilities:
Delivers expert rehabilitation services in accordance with medical directives.

Reassesses clients to determine ongoing rehabilitation requirements, collaborating with physicians and the healthcare team to adjust care plans as necessary.

Evaluates patient needs, formulates, assesses, and modifies care strategies to achieve objectives of Rehabilitation Interventions.

Works with multidisciplinary teams to set and review collective goals for each patient.

Educates patients, families, and caregivers on the comprehensive rehabilitation program.

Consults and guides team members on specific rehabilitation techniques and practices.

Monitors and communicates patient progress and any changes in condition to the physician and relevant team members.

Refers patients to other specialists as appropriate and approved by the physician.

Teaches new skills or retrains clients in daily activities that have become challenging due to health issues.

Identifies equipment needs to support independent living.

Instructs individuals and/or families in the use of adaptive devices to enhance functionality; ensures equipment is well-maintained.

Trains individuals and/or families in the use of prosthetic and orthotic devices.

Conducts work capacity evaluations and may refer individuals to specialized vocational training as per policies and regulations.

Ensures accurate and timely documentation in accordance with the care plan.

Participates in care meetings or other collaborative discussions to ensure cohesive home health services (if applicable).

Prepares and submits clinical progress summaries based on goal achievements.

Provides educational sessions for various disciplines as required.

Engages in peer consultation processes.

Supervises Rehabilitation Assistants in line with company policies and state regulations.

Involves in discharge planning when necessary.

Adheres to local, state, and federal regulations regarding professional standards and practices.

May create splints and instruct individuals and/or families in their use and benefits.

Performs additional duties as assigned or necessary.

Minimum Qualifications:
Current licensure as a Rehabilitation Specialist in the relevant state.

Graduation from an accredited program in Rehabilitation Therapy.

Preferred: One (1) year of prior professional experience in Rehabilitation Therapy.

Compliance with all relevant professional practice standards.

Participation and completion of Amergis' Competency program when applicable.

Current CPR certification if applicable.

TB Questionnaire, PPD, or chest x-ray if applicable.

Current Health certificate as per contract or state regulations.

Must meet all federal, state, and local requirements.

Successful completion of new hire training as applicable to job site.

Understanding of patient confidentiality and HIPAA regulations.

Ability to effectively communicate information to and from relevant individuals (including supervisors, co-workers, clients) through strong communication skills; proficiency in English is required.

Computer proficiency is necessary.

Must be at least 18 years of age.

Must have at least 2 years of experience in med-surg/tele.
