Mental Health Care Coordinator Professional

About the Role- Provide comprehensive care coordination services to children and families, ensuring support is family-focused and inclusive.
- Act as a liaison with various agencies and programs to establish and maintain supportive relationships for the growth of children and families.
- Identify and actively support youth and families in accessing necessary services, including educational, social, therapeutic, and other essential services from formal and informal supports, as well as community-based agencies, services, and organizations.
- Conduct initial diagnostic clinical assessments and evaluations of new clients and families, and develop, implement, and monitor individualized care plans.
- Maintain collaborative relationships with local service providers to facilitate referrals and ensure effective care coordination.
- Regularly communicate with clients, collaterals, and supportive service providers, including via phone calls, scheduling, and attendance at team meetings, to facilitate communication among various providers.
- Prepare and submit written assessments, progress notes, treatment planning, and safety planning in a timely manner.
