About the Job Opportunity

We are excited to offer a challenging and rewarding Compassionate Care Coordinator Specialist position at Riverside Community Care in Needham. This role requires a highly skilled and empathetic professional to provide comprehensive care coordination services to children and families.

The ideal candidate will be able to work collaboratively with various agencies and programs to establish and maintain supportive relationships for the growth of children and families. Additionally, they will be able to identify and actively support youth and families in accessing necessary services.

About the Responsibilities
  • Develop and implement individualized care plans for children and families, ensuring support is family-focused and inclusive.
  • Act as a liaison with various agencies and programs to establish and maintain supportive relationships for the growth of children and families.
  • Maintain collaborative relationships with local service providers to facilitate referrals and ensure effective care coordination.
  • Prepare and submit written assessments, progress notes, treatment planning, and safety planning in a timely manner.
