About the Intensive Family Support Services Coordinator Position

We are seeking a highly skilled and compassionate Intensive Family Support Services Coordinator to join our team in Needham. This role offers a unique opportunity to make a positive impact on the lives of children and families by providing comprehensive care coordination services.

The ideal candidate will be able to work collaboratively with various agencies and programs to establish and maintain supportive relationships for the growth of children and families. Additionally, they will be able to identify and actively support youth and families in accessing necessary services.

About the Job Description
  • Provide comprehensive care coordination services to children and families, ensuring support is family-focused and inclusive.
  • Act as a liaison with various agencies and programs to establish and maintain supportive relationships for the growth of children and families.
  • Identify and actively support youth and families in accessing necessary services, including educational, social, therapeutic, and other essential services from formal and informal supports, as well as community-based agencies, services, and organizations.
  • Conduct initial diagnostic clinical assessments and evaluations of new clients and families, and develop, implement, and monitor individualized care plans.
