Hospice Care Nurse

Job Summary
VIA Health Partners is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join our team in providing end-of-life care to patients and their families. As a LPN, you will play a vital role in delivering high-quality patient care and supporting our interdisciplinary team in achieving our mission.
Key Responsibilities
- Deliver patient care according to established plans of care, working collaboratively with registered nurses and other healthcare professionals.
- Conduct thorough assessments of patients' physical and mental health, documenting findings and communicating with the care team.
- Participate in interdisciplinary group conferences to ensure seamless care coordination and patient-centered care.
- Develop and maintain accurate and timely clinical documentation, adhering to VIA Health Partners' guidelines and regulatory requirements.
- Stay up-to-date with current trends and best practices in hospice and palliative care, applying new knowledge and skills to improve patient outcomes.
Requirements
- A current, active, and unrestricted compact license as a Licensed Practical Nurse (LPN) is required.
- Minimum 1-year related experience in direct care of patients with life-limiting illnesses, preferably in hospice and palliative care.
- Computer proficiency and ability to maintain accurate records.
