Position Overview

Vertis Therapy is seeking a committed PRN Physical Therapist to join our team. Our organization is dedicated to providing exceptional therapy services in a supportive environment.

The Physical Therapist is responsible for assessing and treating patients based on physician directives, utilizing therapeutic methods aimed at enhancing physical capabilities and promoting independence, safety, and overall quality of life. This role includes overseeing the delivery of care throughout the treatment process and guiding the Physical Therapist's Assistant in their responsibilities.

Main Responsibilities:

  • Analyze patient information to inform assessment and treatment strategies.
  • Conduct comprehensive functional assessments using standardized methods, addressing home, vocational, and equipment needs.
  • Create and execute tailored treatment plans that align with patient goals.
  • Adhere to established evaluation, treatment, and documentation protocols.
  • Document patient progress at least every seven days.
  • Adjust treatment plans as necessary throughout the care continuum.
  • Maintain clear and concise documentation of treatment, including discharge summaries.
  • Guide Assistants in treatment methodologies and supervise their interactions with patients.
  • Introduce patients to program services and treatment protocols relevant to their conditions, ensuring they are aware of any associated risks.
  • Collaborate with nursing staff, patients, and families to optimize patient functional abilities.
  • Educate patients and their families on exercises and therapeutic practices to continue post-discharge.
  • Assist in the referral process for additional services needed after discharge, staying informed about community resources.
  • Engage in rehabilitation team meetings to discuss patient progress and needs.
  • Participate in departmental meetings as required.
  • Pursue continuing education opportunities to maintain clinical expertise.
  • Contribute to quality improvement initiatives by responding to audits and satisfaction surveys.
  • Comply with Vertis Therapy Management policies, as well as state and federal regulations.
  • Provide feedback to the Rehab Director regarding the performance of assistants when requested.
  • Promote Vertis Therapy's programs and services through community engagement.
  • Perform additional duties as assigned.

Qualifications:

  • Master's or Bachelor's degree in Physical Therapy with relevant experience.
  • Current licensure in the state of practice.
  • Preferred experience in rehabilitation settings.
  • Membership in the American Physical Therapy Association is advantageous.
  • Proven skills in specialized areas of practice.
  • Strong communication and interpersonal abilities.
  • Proficient computer skills for documentation and job functions.
  • Knowledge of physical therapy principles, methods, and equipment.
  • Understanding of accreditation standards and compliance obligations.
