Responsibilities:

- Provide direct patient care, including bathing, dressing, and feeding assistance
- Assist with daily living activities, such as transferring, toileting, and ambulating
- Collaborate with the care team to develop and implement personalized care plans
- Maintain accurate and timely documentation of patient information

Requirements:

- Must be 18 years of age or older
- High School Diploma or Equivalent preferred
- Obtain CNA certification within 120 days of hire
- Med Aide certification required if eligible
- Ability to work effectively in a fast-paced environment, prioritizing tasks and managing time efficiently
