Job Overview

Job Description

This role involves providing client care, service coordination, and facility safety plans for interim housing clients under the direction of the Pathway Home Program Manager/Supervisor. The Resident Monitor will provide client assistance daily and collaborate with program staff to ensure seamless delivery of services.

The successful candidate will have a strong understanding of LAHSA Funded Programs and their eligibility criteria. They will be responsible for developing individual housing service plans, making linkages, and securing needed services for participants. Additionally, they will assist in developing and implementing client life-skills needs and coordinate with other project staff and partners to provide housing placement services and supportive services.

This position requires excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment. The ideal candidate will be highly organized, detail-oriented, and able to maintain confidentiality and professionalism at all times.

Key Responsibilities:
  • Provide client care and service coordination for interim housing clients
  • Develop and implement client life-skills needs
  • Coordinate with other project staff and partners to provide housing placement services and supportive services
  • Assist in developing and maintaining accurate documentation in client files
  • Work collaboratively with program staff to ensure seamless delivery of services
Requirements and Qualifications

To be considered for this role, applicants must have an Associate degree in systems, Social Services, or other administrative or business sector from an accredited or state-approved college or university. A minimum of 1-2 years of job-related experience is required, as well as high proficiency in HMIS, Microsoft Word, Excel, Access, and other database programs. The ideal candidate will also have excellent communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.

A valid California Driver's License and proof of insurance are required, as well as reliable transportation. Verification of Employment Eligibility and Background Check is also mandatory. TB test results must be no more than three months prior to the date of hire, and CPR and First Aid certifications must be obtained within 30 days of employment.

Salary and Benefits

The salary for this position is estimated to be between $22.50hr to $24.00 per hour, depending on experience. Special Service for Groups, Inc- HOPICS offers a competitive benefits package, including medical, dental, and vision insurance, as well as retirement savings options.
