Job Summary:

Provide supportive services to Veterans and their families, utilizing the critical time intervention strength-based case management approach to help them break the cycle of crisis and vulnerability in their lives. Work collaboratively with community resources to ensure sustainable permanent housing interventions.

Key Responsibilities:
  • Maintain positive work atmosphere through effective conflict management and professional communication practices.
  • Develop and implement case plans with participants, including short- and long-term goals.
  • Provide linkages to community services, including court appointments, landlord/tenant conflict resolution, and educational activities.
  • Ensure client confidentiality and maintain comprehensive demographic data on all participants.
  • Submit monthly summary of service statistics to direct supervisor and/or SSVF Program Manager.
Requirements:
  • Bachelor's degree in social work, public health, and/or human services, or previous experience working with the homeless population and/or Veterans.
  • Pass a pre-employment background inquiry and annual motor vehicle check with a valid driver's license.
  • Excellent written and verbal communication skills, including experience or capacity to demonstrate positive interpersonal skills and work as a cooperative team member.
  • Proficiency in computer software applications, including email, MS Office applications, Internet, and Social Media platforms.
