Case Manager RN- Field

CVS Health is seeking a dedicated and compassionate Case Manager RN to join our team. As a key member of our Aetna team, you will play a vital role in delivering exceptional patient care to our dual eligible populations.
Program Overview
Our Case Manager RN will work closely with members, providers, and community organizations to address the full continuum of our members' health care and social determinant needs. With a focus on compassion and excellence, you will facilitate the delivery of appropriate benefits and/or healthcare information, promoting wellness activities and developing strategies to ensure timely return to work.
Key Responsibilities
- Act as a liaison with members, employers, providers, insurance companies, and healthcare personnel as appropriate.
- Implement and coordinate case management activities, including consultant referrals, home care visits, and alternative levels of care.
- Assess and analyze members' medical and/or vocational status, developing a plan of care to optimize wellness and medical outcomes.
- Communicate with members and stakeholders, preparing required documentation of case work activities.
- Interact and consult with internal multidisciplinary teams to maximize best health outcomes.
Requirements
- Minimum 3+ years clinical practical experience, with preference for Case Management, discharge planning, and/or home health care coordination experience.
- Ability to travel 30-60% within designated counties.
- Active and unrestricted licensure in IL.
- Preferred qualifications include excellent analytical and problem-solving skills, effective communication and interpersonal skills, and proficiency with standard corporate software applications.
