Key Responsibilities- Assist in the planning, implementation, and coordination of patient care services to ensure seamless delivery of care.
- Collaborate with patients, families, and healthcare teams to develop and implement individualized care plans.
- Provide education and support to patients and families on end-of-life care, pain management, and symptom control.
- Conduct regular assessments and evaluations to ensure patients' needs are met and care plans are updated accordingly.
- Current New York State Registered Nurse License
- 2 years of experience in nursing, with a preference for 1 year of community hospice or home health experience
- Associate's or Bachelor's Degree in Nursing
