Overview

VNS Health Hospice RNs deliver comprehensive end-of-life care to patients in the comfort of their own homes, surrounded by loved ones. Our interdisciplinary team, including dieticians, spiritual care counselors, and bereavement specialists, works together to provide personalized care plans that address the physical, emotional, and spiritual needs of patients and their families. As a Hospice RN, you will have the opportunity to make a meaningful impact on the lives of others while working in a dynamic and supportive environment.

Responsibilities
  • Deliver high-quality, patient-centered care to patients and their families in their homes
  • Develop and implement individualized care plans that address the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication
  • Provide education and support to patients and their families on topics such as disease progression, wound care, and medication management
  • Document patient progress and outcomes in a timely and accurate manner
Requirements
  • Current license to practice as a Registered Nurse in New York State
  • Minimum of one year nursing experience in a medical, surgical, or hospice environment
  • Valid driver's license or NYS Non-Driver photo ID card may be required
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and as part of a team
