Job Summary

We are seeking a skilled Home Health Aide to join our team. As a Home Health Aide, you will provide personal care and support to patients in their homes, working under the direction of a Registered Nurse.

Responsibilities
  • Provide personal care and support to patients, including bathing, dressing, and grooming
  • Assist patients with daily living activities, such as eating, toileting, and transferring
  • Monitor patients' conditions and report any changes to the Registered Nurse
  • Collaborate with the interdisciplinary team to develop and implement patient care plans
Qualifications
  • Successful completion of a Home Health Aide Certification program recognized by the New York State Department of Education
  • Registration with the NYS DOH Home Care Registry
  • High School Diploma or equivalent
  • Ability to read and write English at a 10th grade level
  • Ability to perform math at a 9th grade level
  • Basic computer skills
  • Effective interpersonal skills
